New HCM Support Program from Mended Hearts Inc
Welcome Home Hypertrophic Cardiomyopathy (HCM): Is a new program we are launching, and we need your help getting the word out to your contacts at your local hospitals AND your visitors. Welcome Home HCM is a free program that will provide email and one-on-one call support to patients who have been newly diagnosed with HCM or who have recently been in the hospital and are transitioning home again. In addition to weekly educational and emotional support emails, the patient also receives bi-weekly calls from one of our HCM accredited visitors to help support them.
If you know of anyone at your hospital who would be willing to talk to us about the program and how they can share it with their patients, OR you and your visitors would like to speak with us about the program, please send me an email at mandy.sandkuhler@mendedhearts.org, we would love to tell you more.
